Welcome to the Onboarding Forms Tutorial!
This tutorial will cover the essentials with onboarding forms! Including how to assign your forms correctly, how to customize your forms, and how each type of form functions.

Key points about onboarding forms
- Forms are assigned at the time an appointment is scheduled, based on the Patient type, and the Service type.
- You can mix and match these combinations for each form. For example, if you would like a HIPAA Acknowledgement form to be assigned only to patients scheduled for a particular service, you can select that accordingly.
Medical Questionnaires
- Your medical questionnaires are interactive. This means that your patients will be able to click into these forms, do pain scales, and type their answers to questions.
- Medical Questionnaires must be built within the system however, so you will not be able to upload word documents or PDFs as Medical Questionnaires.

Read & Sign Forms
- With Read & Sign forms (otherwise known as consent forms), patients will be able to read through each form listed, consent, and then add their own personal signature. That signature will then be added to the end of the form, and can be viewed in Patient File > Forms.
- Keep in mind that Read & Sign forms are not interactive in the system, meaning that your patients will not be able to click into or write on these forms.
Finally, you can also download consent forms from our Help Center by clicking HERE! These forms are for reference only, so the clinic is ultimately responsible for ensuring this form meets local health/acupuncture regulations.
Health Screening Forms
Your screening forms are interactive. This means that your patients will be able to click into these forms, do pain scales, and type their answers to questions.
- Health Screening forms must be built within the system however, so you will not be able to upload word documents or PDFs as Health Screening Forms
- Your Health Screening Forms will become available to your patients within the time frame set within Clinic Settings > Notification Settings > Screening Forms.
