Does Unified Practice Provide Consent Forms For Download?
Important: You Must Read Before Continuing or Downloading These Forms
The material and information contained in these forms is for general information purposes only. You should not rely upon the material or information on the forms provided as a basis for making any business, legal or any other decisions. Unified Practice makes no guarantee about the completeness, accuracy, reliability, suitability or availability with respect to the information contained in these forms. Any reliance you place on such material is therefore strictly at your own risk.
We often get asked "Can you provide me with consent forms?" The answer is: We normally shy away from providing legal documents meant to protect you and your patients. To address this common question, we've decided to post examples of consent forms used by other clinics available for you to download.
These forms are for reference only and are not intended to substitute legal forms provided to you by an attorney or legal council.
We have provided the following example of forms in MS Word format below:
- "HIPAA_Acknowledgement"
- "Informed Consent"
- "Welcome Letter"
Download the sample forms HERE.
Once you download and update the sample forms with your clinic's information, save the files as a PDF and you can upload as Read & Sign Forms via the Onboarding Forms settings page.
