Learn what Patient information Unified Practice collects
Unified Practice will collect the following information from Patients during the New Patient onboarding process on the Web and iPad.
You do not need to add these fields in the Medical History forms via the form builder.
- Patient Personal Information and Demographics: Name, DOB, gender, occupation, marital status, race, ethnicity
- Patients Contact Information: Phone, Email, Address,
- Emergency Contact Information: Contact Name, Phone, Email, Relationship
- Insurance Information
- Medical: Medications, Vitamins, Allergies and Medical Devices
- Primary Physician's contact information, Current conditions & therapies by physician
