University Edition: SOAP Note Statuses and What They Mean.

Unified Practice allows student interns to sign a SOAP note and submit it to a supervising clinician for approval. Below you will find a description of the SOAP signature and approval process for student interns.



Intern SOAP statuses and what they mean:

  • Begin Intake: Displayed in black font. This indicates that a SOAP note has not been started for this patient. When clicked the SOAP note note will open in edit mode. The student will be able to add or edit information for the appointment.
  • Continue Intake: Displayed in black font. This indicates that a SOAP note has been started for the visit. When clicked the SOAP note will open in edit mode and the intern may continue adding or editing information. 
  • In Review: Displayed in dark yellow font. This status indicates that the intern has signed the SOAP note and submitted it to the supervising clinician for review. When clicked the SOAP note will open in view-only mode and cannot be edited by the student.
  • Rejected: Displayed in red font. This status indicates that the supervising clinician is returning the SOAP note to the intern to edit. When clicked the SOAP note will open in edit mode. From here the intern can again sign and submit the chart for review.
  • Approved: Displayed in green font. This status indicates that the SOAP note is complete and signed. When clicked the SOAP note will display in view-only mode. The intern may now add amendments to the note if necessary.



Supervisor SOAP statuses and what they mean:

  • Not Started: Displayed in black font. The SOAP note has not been started by the intern.
  • Started: Displayed in black font. The intern has clicked Begin Intake and started a SOAP note for the visit.
  • In Review: Displayed in yellow font. This status indicates that the student has signed the SOAP note and submitted it for supervisor approval. When clicked the SOAP note will open in approve/reject mode. 
  • Rejected: Displayed in red font. This status indicates that the supervisor has rejected the SOAP note and returned it to the intern for edits. When clicked the note will display in edit mode.
  • Approved: Displayed in green font. This status indicates that the SOAP note has been approved by the supervisor. The note is now signed. When clicked, the note will display in view-only mode. Clinician and intern may now add amendments to the SOAP note if necessary.
