      What Can We Import For You?

      The Unified Practice team can help you with custom medical forms creation, data migration services, and expert training. We're here to help and we're at your service.       Quotes for Patient-Related Insurance Details, Patient Balance, Patient On-boarding Medical Questionnaires, and Red Flag Notes will be provided by the team after the requested files have been reviewed. 

      Data Migration Services

      Want to have Unified Practice import your data or want to import more than patient contact and demographic info? Unified Practice Data Import services can import patient contact information, basic demographics, inventory lists, fee schedules, and insurance lists. We are able to complete most projects within 3-5 business days. You can provide your data as a Comma–Separated Value (.CSV) or Microsoft Excel (.XLS) file, depending on what you would like to import.

       

      Patient Contact and Demographic Information (Free). The template for structuring your patient data for this import is downloadable here. Fields that can be imported:

      • Phone 1 (Cell)

      Inventory (Free). The template for structuring your inventory data for this import is downloadable here. Below is a general outline of required and optional column headings for the inventory upload .csv file. Items in bold are required.

      • Item Name
      • Item Code
      • Category - if no Category is provided, all items will be put into one Category
      • Medium - raw, granule, patent, capsule, etc.
      • Tax Rate (0.0000%)
      • Unit of Measurement (UOM) - possible values for Unit of Measurement (UOM) are - g (gram), lb (pound), bt (bottle), bx (box), ml (milliliter), oz (ounce), p (piece).
      • Quantity on Hand (QOH) - if no QOH is provided, it will be set to 0.
      • Cost Per Unit ($00.00) - the cost you pay to purchase the item
      • Price Per Unit ($00.00) - the cost your patients pay to buy the item from you
      • Pinyin Name Simple (no accents)

      Medical Forms Creation - Patient On-boarding Medical Questionnaires ($45 and up)

      • Unified Practice provides the tools to create or customize your own medical history forms. However, if you would like a form built and don’t want to do it yourself, our Custom Form Creation Service can handle everything. If you would like our team to create Medical Questionnaires based on ones you already use, reach out to support@unifiedpractice.com and our team will review the files and provide a quote for creating the form.
