How do I Bill Telehealth/Telemedicine Services?

Here's a helpful list of CPT Codes for billing Telehealth appointments. 

Telehealth/Telemedicine services will need to be charted and billed appropriately. Since these visits are not in person, they have their own set of codes and approximate pricing in order to accurately bill patients and/or their insurance companies.

Telemedicine visit need to be documented just as a direct contact visit via EHR or chart notes would be. It’s helpful to include start and stop time of call, what was discussed, pain scale, diagnosis, etc.⁣



CPT Codes for Online Services:

99421: Online digital E&M, up to 7 days, 5-10 minutes
(price would be approx. 40% of your rate for 97810/first 15 mins of Acupuncture)⁣

99422: Online digital E&M, up to 7 days, 11-20 minutes
(price would be approx. 85% of your rate for 97810/first 15 mins of Acupuncture)⁣

99423: Online digital E&M, up to 7 days, 21 or more minutes
(price would be approx. 140% of your rate for 97810/first 15 mins of Acupuncture)⁣

  • Pricing approximations listed here are suggestions, and actual pricing can differ by state and region. 

  • This should not be used for test results, scheduling appointments, or other communication that does not include E&M.
  • “Up to 7 days” means: online digital evaluation management service for an established patient for up to seven days of cumulative time during a seven day period, for five to 10 minutes. So this may mean several communications with a patient within a seven day time period, and the time would be cumulative. If you only have one visit during the seven days cumulative time, you can still bill for just this one.⁣
  • Also, if these codes are used for a condition you were already treating the patient for, it must be billed no sooner than 7 days after your last in office E&M. If it is a new problem/condition, you can bill at any time.⁣


 CPT Codes for Telephone Calls:⁣

99441: 5-10 minutes of medical discussion
(price would be approx. 40% of your rate for 97810/first 15 mins of Acupuncture)⁣

99442: 11-20 minutes of medical discussion
(price would be approx. 80% of your rate for 97810/first 15 mins of Acupuncture)⁣

99443: 21-30 minutes of medical discussion
(price would be approx. 115% of your rate for 97810/first 15 mins of Acupuncture)⁣
