Managing Onboarding Forms

How to create/upload/edit/delete the forms that your patients fill out.

There are three types of onboarding forms:

  1. Medical Questionnaires
  2. Read & Sign Forms (Consent Forms)
  3. Screening Forms

Important Note: We do have questions regarding Personal and Demographic information already built within the system. These will automatically be sent out through the Onboarding workflow and the answers will display within the Patient File > Personal Tab > Medical Tab. These questions are not customizable. 


Medical Questionnaires:

These are interactive forms. Patients will be able to answer questions by checking off boxes, typing answers, using pain scales and more. Any information that you would like to collect from your patients other than the built-in personal and demographic questions must be created using the Form Builder.

In Clinic Settings > Onboarding Forms, you can manually add medical forms by clicking on "Add Form" in the top right corner. A blank worksheet will appear with template controls which you can drag, drop & customize to create your own custom form. This enables patients to fill in the inputs you created for them, allowing you to have a log of their Health History and other Medical data. You may edit forms at any time. However, changes will not be reflected for the patient for changes made after the form was assigned.  Forms are assigned at the time the appointment is created on the calendar.

Read & Sign Forms:


Forms such as your Consent to Treat, HIPAA forms, Clinic Policies, and Financial Agreement that your patients need to sign but not otherwise contribute to belong in this section. Patients will not be able to write directly on these forms. Once a patient has reviewed all of the documents in this section, they will be able to give their electronic signature which is then applied to all forms in the section that have been assigned.

You will upload a PDF document for Read & Sign forms and assign it to the appropriate patient and appointment type. This is done in Clinic Settings > Onboarding Forms by clicking on "Upload PDF" in the top right corner.

Screening Forms:

Screening forms are interactive forms sent for each and every appointment for the appointment type(s) they are assigned to. They are useful for information you want to collect at or before each visit, such as COVID-19 screening or pain scales.  Patients will be able to answer questions by checking off boxes, typing answers, and providing a signature at the bottom. You can manually add screening forms by clicking on "Add Form" in the top right corner. A blank worksheet will appear with template controls in the right grid which you can drag, drop & customize to create your own custom form.

Onboarding forms can be edited or made inactive from Clinic Settings > Onboarding Forms. Forms are assigned to patients when appointments are booked, based on the type of patient (New or Existing) and the Service for which they book.

Forms already associated to and filled in by patients will remain in their respective files regardless of their active status, but new forms can always be assigned to patients with existing forms.
