Chief Complaint & ICD-10 SOAP note sections

There is a difference between how the ICD and Chief Complaints are categorized on the web platform versus the iPad charting app. Learn how to add and use both!


Unified Practice EHR for TCM App for Charting

On the iPad, you have the ability to enter both ICD and Chief Complaints (CC's) within the Chief Complaint and ICD Codes section by tapping in the top right corner.

ICD-10 codes will be listed with an alphanumerical code associated to them.

To add a Chief Complaint, simply type in and click add without associating a code. These will appear in the note as CC 1, CC 2, etc. (seen in the photo below).

 

 

 

After you've added your ICDs while charting on the iPad, ICD codes will display in the Billing Info page (on the web platform) for that appointment so Super bills and Claims can be generated with this information.

 

 

Web Platform Charting

You can view or add ICDs and Chief Complaints within the SOAP note on the web platform.  The Chief Complaint section is located near the top of the chart, while the ICD section is located separately on the bottom of the chart.

Troubleshooting: "I entered an ICD code into the Chief Complaint section of the SOAP note on the iPad, and now that information is not showing on the SOAP note on the web. What happened?"

  • These fields are separate and used differently during charting.

  • ICD codes are diagnosis codes used for billing purposes.

  • Chief Complaints are concise statements describing the symptoms, problems, or other reasons for the patient's visit.
